• type of performing organization represented
• dates and locations of all performances
• when materials are needed
• whether organization is licensed by ASCAP or BMI
• number of string parts required
• name of conductor
• type of concert (subscription, young people's, etc.)
• whether concert will be taped for live or future broadcast or for commercial distribution
